Church Membership Questionnaire

1 Peter 5:2 calls pastors to "shepherd the flock of God among you" and Christians to "obey your leaders and submit to them, since they keep watch over your souls as those who will give an account". Romans 12:5 describes believers as "one body in Christ and individually members of one another".

This questionnaire is designed to help the pastors of Rooted get to know prospective members of the church so that they can shepherd well. It is also an opportunity for you to ask questions around what it would mean to commit to this local body of believers called Rooted Church.
